Forms (for Division F accumulation members): Use this form to:
Change of Details Form
  • Change your personal details
  • Request online transaction access to FirstNet
TFN Notification Form
  • Notify the OSF of your Tax File Number
Non-Lapsing Death Benefit Nomination
  • Nominate one or more beneficiaries to receive your super in the event of your death
  • Update or revoke your non-lapsing death benefit nomination
Investment Selection Form

Note: You can also make these transactions online using FirstNet if you have transaction access.

  • Change or switch your investment selection
  • Select the investment option to deduct fees and/or insurance premiums from (if applicable)
Additional Investment Form
  • Make a personal (after tax) contribution to your account by cheque
  • Make a spouse contribution to a Division F account
Direct Debit Request Form
  • Make a personal (after tax) contribution to your account. A separate request must be completed for each direct debit payment.
Request to transfer
  • Transfer money from another fund into your OSF account
  • If you are transferring your entire account balance, you may be eligible to transfer any insurance cover you have in your other fund – refer to ‘Transfer insurance cover’ below

Transfer insurance cover
  • If you are transferring your entire account balance into your Division F account, you may be eligible to transfer any insurance cover you have in your other fund – this document includes both a Request to Transfer (for your account balance) and a Transfer of Insurance Cover form
Life Events Insurance Cover Option
  • Apply to increase existing Death and TPD or Death–only insurance cover under a ‘life event’ option
Identification and Verification Form
  • If you joined the OSF on or after 12 December 2007, we must verify your identity for the purposes of Anti-Money Laundering and Counter-Terrorism Financing laws before any benefit can be paid to you in cash- refer to the Product Disclosure Statement for more information.
Withdrawal Form
  • Withdraw a lump sum from your account (conditions may apply)
  • Transfer money out of the OSF into another fund

Withdrawal Form - Temporary Residents Only
  • Withdraw a lump sum from your account if you are or were a temporary resident and have now left Australia
Application for Division F Account
  • You can use this form to open a Division F account if you are:
    • an employee of a Commonwealth Bank ‘Associated Employer’ (ie. a wholly-owned subsidiary of Commonwealth Bank of Australia)
    • a member of another OSF division (eg. account-based pension or defined benefit division)
    • the spouse of a current OSF member.
    For more information about Division F, refer to the product disclosure statement (Note: If you are a current Group employee who wants to join Division F, you do not need to complete this form - refer to the PDS for more information.)

Contributions from external employers
Division F Members If you are a Division F Member, you can have superannuation contributions from external employers (ie. employers other than the Commonwealth Bank Group) made to your account. If you would like to begin this process, you should give an OSF External Employer Information Sheet to your new employer as soon as possible. This Information Sheet provides your employer with some basic information about using the OSF so that they are aware of what they need to do in order to satisfy their superannuation guarantee obligations by making contributions on your behalf.
External Employers If you are an employer of an OSF Division F member who has requested that you make superannuation contributions to their OSF account, you should read and print a copy of the OSF External Employer Information Kit, which includes all of the documentation you will need to help you meet your superannuation guarantee (SG) obligations by contributing to the OSF. You will need to print this Kit before proceeding to use the OSF to meet your SG obligations.
