- Make a personal (after tax) contribution to your account. A separate request must be completed for each direct debit payment.

- If you are transferring your entire account balance, you may be eligible to transfer any insurance cover you have in your other fund – refer to ‘Transfer insurance cover’ below

| Identification and Verification Form |
- If you joined the OSF on or after 12 December 2007, we must verify your identity for the purposes of Anti-Money Laundering and Counter-Terrorism Financing laws before any benefit can be paid to you in cash- refer to the Product Disclosure Statement for more information.

- Withdraw a lump sum from your account (conditions may apply)
- Transfer money out of the OSF into another fund

- Withdraw a lump sum from your account if you are or were a temporary resident and have now left Australia

| Application for Division F Account |
- You can use this form to open a Division F account if you are:
- an employee of a Commonwealth Bank ‘Associated Employer’ (ie. a wholly-owned subsidiary of Commonwealth Bank of Australia)
- a member of another OSF division (eg. account-based pension or defined benefit division)
- the spouse of a current OSF member.
